Telehealth Consent
Telehealth means receiving health care from a provider who is not in the same place as you, using electronic communication such as secure messaging, questionnaires, phone, or video. By accepting this consent, you agree to receive care from independent, licensed Providers through the Aldwyn platform.
What to expect
- A Provider licensed in your state will review your intake and may contact you with questions, request labs, or ask for a video visit.
- Care may be delivered asynchronously, meaning your Provider reviews your information and responds without a live visit, where your state allows it.
- Your Provider decides whether telehealth is appropriate for you and may refer you to in-person care.
Benefits and risks
Telehealth can make care faster and more convenient. Risks include the possibility that information may be incomplete, that technical problems may delay care, that a Provider may not be able to perform a physical exam, and that, despite safeguards, security protections could fail.
Your responsibilities
- Give complete and accurate information about your health and medications.
- Tell your Provider about side effects or changes in your health.
- Keep a primary care provider for ongoing and in-person care.
- Call 911 or go to the nearest emergency room for any emergency.
Your rights
You may withdraw this consent or stop telehealth care at any time without affecting your right to future care. You have the same privacy rights as with in-person care, described in our Notice of Privacy Practices.
